Fundraising & Event Forms

Host an Event or Fundraiser at VHTPA

To host a fundraiser or on-campus event, students and sponsors must complete the following three essential forms and submit them at least three weeks in advance of the desired event date:


Required Forms

  1. Event Calendar Form (Updated Nov. 2024)

    • Required for all on-campus activities and events

    • Ensures events are scheduled without conflict and that proper administrative/staff supervision is arranged

    • Must be approved three weeks prior to the event date

  2. VHTPA Fundraising Request Form (Updated Nov. 2024)

    • Required for any fundraising activity by an approved club

    • Must be submitted along with the Event Calendar Form and the LAUSD Request for Authorization Form

    • Must be completed by the club sponsor with a clear and detailed description of the fundraiser

    • Additional forms may be required for off-campus fundraising—consult Financial Manager Sandy Lam in the Student Store for guidance

  3. LAUSD Request for Authorization Form

    • LAUSD requires this form for any fundraising activity to be officially approved

    • Must accompany both the Event Calendar Form and Fundraising Request Form

    • Completed by the sponsor with detailed activity information

    • For off-campus fundraising, check with the Financial Manager for additional documentation

Important Notes

  • All three forms must be submitted together at least three weeks in advance to allow time for administrative and district-level approval.

  • LAUSD makes the final decision on all fundraiser approvals. You may track the approval status online before launching your event:

Host an event
  1. Event Calendar Form
 
Host a fundraiser, students should follow these three essential steps:
  1. Event Calendar Form
  2. VHTPA Fundraiser form
  3. LAUSD Request for authorization form

Description of forms: 

 
  • Event Calendar Form (updated Nov. 2024)
    • In order to host any activity on campus, you must complete this form and get it approved at least three weeks before the desired date.
    • The purpose of this form is to prevent the overlapping of events as well as to ensure administrator and staff supervision for the event.

  • VHTPA Fundraising Request Form   (updated Nov. 2024)
    • In order to fundraise, approved clubs must submit this form along with the Event Calendar Form AND the Request for Authorization form (see attachment below) at least three weeks before the start of the fundraiser to be on the safe side. Forms should be detailed in description and filled out by the Sponsor. [There are additional forms needed if filling out off-campus fundraising events. Please see Financial Manager Sandy Lam in the student store for more details.

  • LAUSD Request for Authorization 
    • In order to fundraise, approved clubs must submit this form along with the Event Calendar Form AND the Fundraising Request form (see attachment below) at least three weeks before the start of the fundraiser to be on the safe side.
